It's easy to schedule an appoint with a physician at Arizona Oncology. Simply call the office where you wish to have the appointment using our list of physicians and locations.

 

When scheduling your first appointment, you may be asked to provide the following  information about your insurance carrier:

• Insurance company name
• Plan number
• Policy number
• Contact information for claims

First-Time Referral Patients

If another physician has referred you to Arizona Oncology, your first appointment may already be scheduled. If not, you may directly contact the cancer center you prefer or view the list of locations where the physician you were referred to will be available.
